Counseling Feedback Form - Heart Talk Biblical Counseling

Instructions

The Counseling Feedback form is designed to help the counselor improve his/her abilities in counseling. Your willingness to be honest ensures that those who follow you will receive the best possible counseling from God’s Word.

Please answer the following questions with a number “1” through “5.” The number “1” is the lowest rating and “5” is the highest.

A “3” is considered normal and should be given unless there is significant reason to rate the counseling higher or lower. Thank you for completing this form.
